This program is designed to educate providers about the various ambulatory models of palliative care available and to provide education on symptom management for those patients with life limiting illnesses. The aim is to increase utilization of community-based palliative care resources and increase PCP confidence in basic symptom management.

1. Identify and explain the differences between various models of ambulatory hospice and palliative care.
2. Determine when to refer patients to palliative care vs hospice care.
3. Understand and describe basic principles of palliative symptom management for dyspnea and constipation.
4. Explain how to utilize low doze buprenorphine for pain management.
